If you are a first time retail customer please print off a new customer application form, fill it out and fax it along with your completed order on our order form to 1-877-892-3722. We will confirm receiving your order if you put your email address on the order form. After your application has been accepted and approved you will only need to submit your order in the future. Our goal is to protect all legitimate retailers. All invoices upon shipment will be electronically emailed to you.
